Chronic Prostatitis / Chronic Pelvic Pain Syndrome
Harley Street, London
Chronic prostatitis causes persistent pelvic pain, urinary symptoms, and sexual dysfunction, and it can significantly affect quality of life. It’s a common but often poorly understood condition, requiring careful diagnosis and a tailored treatment plan1.
Diagnosis
Assessment starts with a detailed history, physical examination, and targeted tests including urine cultures and validated symptom questionnaires. This identifies the specific subtype and guides treatment.
The four NIH subtypes:
- Acute bacterial prostatitis
- Chronic bacterial prostatitis
- Chronic pelvic pain syndrome (inflammatory or non-inflammatory)
- Asymptomatic inflammatory prostatitis
Treatment
Mr Raison takes an individualised approach, drawing on:
- Antibiotics – targeted therapy (often fluoroquinolones) for bacterial prostatitis
- Alpha-blockers – to ease urinary frequency, urgency, and hesitancy
- Anti-inflammatories – to reduce pain and inflammation
- Pelvic floor physiotherapy – biofeedback and relaxation techniques, delivered with specialist physiotherapists
- Lifestyle changes – exercise, stress management, and diet
- Psychological support – CBT and counselling for the emotional impact of chronic pain
- Advanced phenotyping – pinpointing individual drivers of pain to personalise treatment
This combined approach targets the underlying cause, not just the symptoms, helping patients get back control of their life.
